  • The appeal form must be received or postmarked by the County Registrar's Office no later than 5:00 p.m. on Monday, July 12th, 2010.

  • The form must be filed with three (3) copies to the County Registrar's Office.

  • Please make sure the application is complete. Missing information could delay the appeal process.
